When determining if you're wide set or close set, do you measure the distance between your roots? Or where the tissue begins to project?

Hmm..I used to do the distance between my roots, which led me to believe that I was wide set. Then I put on a bra that fits and realized I was close set. So maybe where the tissue begins to project? Is that what you mean by that? Where they sit in the bra? You might have ninja boobs that look wide set but are secretly close set when supported.
Close-set refers to where the roots of the breast are in relation to one another. Many people confuse "close-set" with having a lot of center fullness. Being close-set requires narrow gores, center fullness requires a lot of volume or open cups near the gore.
