Issue resolved
I tried this bra a few more times and decided that this is still the wrong size. I've decided to try to get fit at a "real" store as soon as I get done with finals this week.
Original problem
I've been wearing the wrong size for as long as I've been wearing bras. This is my latest attempt at finding the right size. I feel like this bra pretty much fits, but it's not giving me enough lift/support and I've got a bit of a saggy look going on. Should I try a different size or style?

helpfulThis looks more like a style issue rather than fit, for the most part. The cups look thin, the straps are at an odd location for that type of cup, and the materials altogether look kind of flimsy (or is this an old bra? Do you wash it by hand?)
As for the fit, the cups don't look quite the right size, and you seem to be tightening the straps too tight to accommodate for lift and support. If you loosened them just a little the cups probably gape open at the top, am I right? So you would need a cup size down, or a larger band but same cup equivalent as the band on this looks too tight, IMO. Although according to your measurements you should be around a size 32 band (is that with the band pulled a little snug, or are you pulling that super tight?) and your 11" cup depth would suggest a size 32F (DDDD) or 34DDD for sister size comparison. Again with measuring correctly, is that the measurement from edge to edge of breast tissue, or are you extending the tape beyond breast tissue at your side? It's important to only measure the breast, but nothing else past that point.

The bra is brand-spanking new haha. I just got it from Bare Necessities yesterday, and they have a pretty reasonable return policy which is great. As for the band size; the measurement listed is with the measuring tape snug, but not so tight that I can't breathe. I have an oddly shaped rib-cage which is part of why the band looks too tight. My ribcage is angled/widened at the top so that it pokes out; even when I was thin it didn't lie flat. I can actually pull the band away from me in the back about 4-6". There's also a pretty big disparity in the size of my breasts; the right one measures at 10" consistently while the left one measures between 10.75"-11" depending on where in my cycle I am. I tried on this bra again and loosened the straps to see if it would gape at the top like you said, but it didn't.
Also I have quite a bit of separated breast tissue under my arms and scars from underwires that were cutting into the side of my breasts. I'm sure this is also affecting my shape somehow, but I'm not sure what to do about it.
Should I exchange this bra for a different size or try an entirely new bra style?
P.S. thank you so much for your in-depth suggestion. I have been so frustrated trying to find bras in the right size. Living in the middle of nowhere sure does have its disadvantages sometimes!

helpfulIf the back of the band is riding up, it's probably too big. But I agree that the cup design is also part of the problem. I feel like cups that don't have any padding/stiffening and lack 3 part construction (see any of Freya or Panache's balcony styles) don't give a lot of lift/support for larger breasts.

helpfulI have very similar measurements to yours, and had the same problem with this bra. I think it is too saggy, I think something that has more support on the side would be better.

helpfulPerhaps try something with a moulded cup. It helps shape the breasts nicely.

Whoops, I meant: is that with the measuring tape pulled tigh, not band...for determining band size.
