helpfulIf the back is riding up and straps are falling off, it's time to try a band size down. Since the gore doesn't tack and the cup depth is so much smaller than your horizontal perimeter, I would suggest trying a larger cup, maybe 40JJ or 40K.
Edit: thanks for adding the side view, now the problem is MUCH more visible! You may even end up wanting to try a 38, that's riding up so far! But perhaps go to 40 first to test the comfort.

helpfulI definitely recommend looking into 40 band bras. Generally speaking, most people whose underbusts are over 36" find a slightly tighter band is better most of the time (the converse situation is true for most people whose underbusts are under 30"; they tend to prefer a slightly looser band most of the time). My guess is that the bra is too tall wired for you. If the outer wires are indeed cutting into your breast tissue at the sides, the you may also need broader wires. 40J might be a better size for you.
***EDIT***
Now that I see the extent to which the band rides, up, I recommend trying both 40J and 38JJ.

she says that the outer wires cut into her breast tissue at the sides, which means that there is a good chance the wires are too narrow.

That part of the fit questionnaire actually says "outer CUP cuts into breast tissue at sides" and new users typically check that box when the wings of the bra/ENDS of the underwires are digging into their armpit. I doubt the wires are really too narrow, they are nearly on her back. The entire bra is riding up due to the loose band but with a better band size this issue may well disappear; if not, it may be addressed with shorter wings or wires.

helpfulFer sure need to try a smaller band, I measure out at a 40/42, but I'm also an h/hh and I buy a 38 in a stretcher bra types because if I buy a loser band the weight of my breasts pulls the band up in the back, even though I technically have the right measurement. Remember 80% of your support comes from the band not the straps. The straps are more there to keep the cups and everything in place, the band does the supporting.

Why do you think the wires are too narrow? It would be nice to actually see a picture of the side of the bra - with your arm out of the way, so we can see where the wire is compared to your breast tissue.
But yes, you need to go down a few band sizes. and honestly the cup looks slightly big to me. It should be too small, looking at your perimeters which are larger than your volume. But it looks too big to me.
