Issue resolved
It helped me recognize the size issue and offered good suggestions for finding a better size.
Original problem
The band is snug but stretchy. Even on at it's tightest, I could still get my hand comfortably under the band. The cups are too big and I don't fill out the tops. Plus the shape of the cups is weird on me, way too high on my chest. Tightening the band helped the fit somewhat but there was still some room in the top of the cups. Empty space is very obvious because it's a full cup. The center seems high, but it lays flat against my sternum. I can't tell if it feels weird because it doesn't fit or if I've just never had bras that laid flat before. The underwire goes too high under my arm and it's not comfortable.
I'm not sure how to determine what a better size would be.

helpfulHm. Well, based on the fact that you can fit your whole hand beneath the band when it's on the tightest hook, I might actually suggest that you try a 38 band. How tight was the tape when you measured 40"? If you measured fairly loose, you might actually find you get the best fit in a 38. (For example, I measure 38 loose and 35.5 comfortably snug, and I take 34 or 36 bands).
It's hard for me to estimate cup size because you are pretty clearly not getting a lot of support from this cup, but that is a combination of band too big and cup too big. It looks to me like you might want to try cups anywhere from 1-3 sizes smaller, as it's too big on the top, bottom AND sides. (Wires too tall is also a classic marker of a too-large cup.).
I see you mentioned on your adventure that you bought these at Nordstrom Rack. Does that mean there's a Nordstrom nearby where you could at least try on a variety of sizes? If so, I'd suggest trying on 38 GG/H/HH and 40 G/GG/H like the reddit folks said. Though if you go to Nordie's they're probably going to want to fit you first, which might not be a bad idea. Sometimes the fitters there are good and sometimes not. Just don't be afraid to ask for alternate sizes if wha they fit you into isn't working.

shayelea Thanks for those suggestions! I'm just starting out with this and probably should have had a better plan before I went shopping.
Also, I think I need to remeasure. 40 inches was the tightest I could go in June, but I've been exercizing more and eating better since then.
