I've since lost 22lbs so the bra is now way too big.
Original problem
Band too tight. Underwire rubs painfully underneath breasts against ribs. Underwire rubs/scratches inner arm as arm moves around because wire is so long and sticks out. Even though cups are big and band is tight gore doesn't lay flat against sternum.

helpfulTo me, the primary fit issue here is that the band is too tight for it to sit directly underneath your breasts, so it keeps sliding down and digging in. This is causing the empty space at the bottom of the cups, lack of space at the top of the cups, and possibly the uncomfortable gore. However, the gore in the Caitlyn is quite wide and if you have close-set breasts, it won't tack regardless of the cup fit.
Until the band is sitting roughly in the correct place, it's a little tricky to figure out if the cup volume is good for you or not. It may be a little big, but it may just be that the bottom of the cup is very empty because the bra is sitting too low. If you have an extender, I'd suggest trying the bra out with that and seeing how it fits if you can get the wires up into your IMF. If you want to try out this bra again in a different size, 40H or 40GG would be likely options. (The same volume, or one cup volume smaller)

helpfulI agree that band is too tight and therefore sits in the wrong place, so, yes, I'd start with 1 size up in band.
As for cups size and gore, it's hard to say at the moment which problems are caused genuinely by them and which ones are side-effects of band currently being too tight.
Center gore could be sitting in the wrong place because it's too high for your shape. A smaller cup most likely would have a lower gore and could resolve this issue.
I believe wrong band size is responsible for the empty fabric at the bottom of the cups and, probably but not necessarily, for the straps feeling too far apart.
On the side view underwire seems to be too wide ending somewhere way beyond you breast edge. It could indicate that cups are too wide and/or too big, but it also can be result of too tight band that distorts the underwires' normal shape. Poking is an expected result in either case.
On the first picture it looks like some of the breast tissues escape the cups creating sideboobs. Maybe it's just a matter of angle at which the picture was taken, but if these sideboobs are present off the camera, then I'd go 1, not 2, cups down for starters, i.e. 38HH--->40H.

helpfulWhen I first tried a caitlyn that was too big in the cup and band size, it felt like the wire was too high under my arm and the material folded over when the wire migrated towards the gore (which didn't tack either). Also, a band size larger gave me that same issue. (a 36g vs a 34gg) going down that size was all it took for the bra to feel good. This might help you on your fit.
