Issue resolved
I was never really able to fix the issue. I tried the smaller band (with extender) with the larger cup which eliminated the painful digging, but then I lost the "push up" part of the push up bra. So for now I've settled on wearing the old bra occasionally for short periods of time. Oh well.
Original problem
Fits good everywhere except where the underwire digs into my sides. I've tried the same bra in a larger size (38D) but the cups were too big (the underwire didn't dig in, but the cups were too big). I like the look of the bra, but it is painful to wear for more than a couple of hours. I don't have a problem with my regular bras of the same size, it only seems to be the push up bras that I have this problem with.

helpfulI am thinking that a smaller band, 32 inches, would suit you better. Measurements and appearance seems to support that. Then you would not have the gore floating off your body while the bra tries to make room for your breasts in a cup that is too small. Once you are in a smaller band, you might then want to loosen the straps so that the band is doing more of the lifting. I am not good at estimating a cup size for you in a 32 inch band but I am sure that others could assist you on that. Smaller band, bigger cup....

helpfulI'm brand new here, so take my words with a grain of salt, but I would second jazzypants that you need a smaller band, bigger cup. Additionally, it looks like your breast roots are wider than this model allows. The underwire appears to cut into your breast tissue a little bit on the sides near your armpit.

helpfulHello. My first post here, but I feel your frustration. The key for you... You want for the length of the bras wires to match your horizontal perimeter measurement or slightly smaller as you like va-voom cleavage. My measurements are similar to yours btw. I used to wear a 34DD because the cups kind of fit. Or a 32DDD (better band (because the cups same are the same size as the 34DD confusingly enough).
My advice for the above bra... The straps are too far apart on your back because you should wear the band on the tightest setting and loosen the straps. More support, less shoulder digging. Yes, the girls will spill out more but your cups need to be much larger. You still have breast tissue which is spilling back and prob makes you think you need a bigger band than you do. If this bra had F cups, you'd find this by swooping and scooping.
I'm guessing here... But based on your measurements, I'd go with a 30H on the loosest band to begin with. Use an extender for the first couple of weeks. After a month you'll be on the loosest setting without it. Several months, the middle. If you want to take it slow, try a 32G. Yep, you look like a G-Unit! 30H = 32G = 34FF = 36F = 38DDD/E all the same cup size. Down a band up a cup usually. You should still get great cleavage with a 30H or the 32G

helpfulOh and don't worry about a bigger cup cutting in more. It took me awhile to get over this idea. Lolol. It won't. As the band will sit lower and be more supportive, the straps will be looser, so the cups won't dig into your sides (currently digging in as they've been pulled up high from strap placement. P. S. If you need a Sports Bra (or in my case my everyday bra) try the Panache Underwire Sports Bra (5021) in a 32G or 32H. A game changer for me!

helpfulThank you ladies for all of your help! If I understand correctly, my breasts would fit in a smaller band with a bigger cup (ex. 36D vs 34DD) because the bigger cup is not only bigger, but also wider (not digging into my sides) and the smaller band will support more so the should straps won't pull up on the cup as much (also less digging into my sides). I always feel like I need the straps as tight as possible because I need the lift - so I tend to keep them very tight.
I do like the va-voom cleavage look so I'm trying to stick with this bra or one like it. I'll give the smaller band/larger cup a try. Thanks again!

You're very welcome. I was where you are now, 3 years ago. I wear 30FF now. No "back fat" issues any more for me, because my breast tissue is where its supposed to be now. Smooth back. Crazy cleavage up front. Looking like Dolly Parton now. Lol. And yep, I'm all natural. If you can get your hands on a 32DDD/E from even VS you'll see the difference. Same great cleavage, no shoulder indentations/ pain or neck pain :) I can even wear halter style bikinis now. It's all about the band first! Happy bra sailing! :)

Va-va-voom options... You're full on bottom like me too... Try balconette styles or ones that have a vertical centre seam. The brand Freya (no affiliation) has really sexy ones... Stay away from their whole Deco line IMO. Not for us full bottomed girls. Lol.

helpfulPush-up bras do what they say, push-up. From the structural engineering point of view, this results in the cups subjected to a downwards rotation momentum that has to be comensated by the lateral portions of the chest-band.
Which needs deep side-panels or the resulting constraints woud stretch the underwires laterally instead of providing an opposite "torque" to balance the cups.
Compared to a bridge structure, I guess you need significantly deeper, aso wider in the back band to comfortably anchor the structure on the body. Wider = more momentum with less pressure.

Do keep in mind GoddessFreya 's recommendation of trying a 32G or 32H bra as sister-sizing the cup upwards from your 36D bra will not give you a large enough cup volume. Good luck in expanding your mind on a cup size that will fit you!
